Our Amherst practice began in 1986 at the Lazar Family Medicine Center behind Millard Fillmore Suburban Hospital. We were delighted to have the opportunity to transition to a new home at Hopkins and Klein in 2008. We welcome our former Lazar patients as well as new patients to our thriving practice. We are working hard to ensure you have both excellent care and an excellent experience with us. We welcome your feedback and partnership.

Our office health care providers include seven faculty family physicians, twelve family physician residents, a clinical psychologist, and our nursing team.

When necessary, we admit and follow our patients at Millard Fillmore Suburban Hospital. In special circumstances, many of our doctors will do home visits.

Appointments

To schedule an appointment, please call 716-688-9641 M-F 8:30am-4:30pm.

Please arrive a few minutes early if you are new to our practice or have changes in your insurance or contact information. Please bring your current health insurance card to each visit. If you are unable to keep your appointment, please call as soon as possible to cancel your appointment.

Health Insurances

Our family physicians accept most major health plans including Independent Health, Blue Cross, Univera, Empire, Medicare, and Medicaid. Prior to your first visit, please notify your insurance carrier that you are changing your primary care provider to a doctor in our practice group. Once you are a patient of our practice, you may see any doctor here, but we encourage you to select a primary doctor from our group and see him/her for all of your visits here to maximize the continuity and quality of your care.

After-hours care

One of our doctors is always on call for urgent problems nights or weekends. Call 688-9641 to reach a physician after hours.

Prescriptions

When your doctor writes a prescription in our electronic health record system, it automatically sends most prescriptions directly to your preferred pharmacy. To avoid running out of continuing medicines, when you pick up your last bottle from the pharmacy (marked “0/no refills”), please have your pharmacy call us to refill the prescription.

Referrals/Authorizations

Please inform us at least five working days in advance of needed referrals, authorizations, forms, or other paperwork. Insurance company procedures require time to complete.
