Developing Our Patient-Centered Medical Home
Sunday, June 28, 2009 at 3:44PM We are currently working with Independent Health and 19 other primary care practices in Western New York as part of a collaborative learning experience to refine our Patient-Centered Medical Home. The goal of this two year (2009-2011) project is to better organize the way primary care offices run to ensure that health care based on the best available evidence is provided consistently. Part of the focus is to improve the design of our practices to make it as easy as possible for the health care team to provide high quality care, and make it as easy as possible for patients to obtain high quality care. A related grant from the US Department of Health and Human Services is allowing us to improve training of our future family doctors in these the approaches.
